School Support Counsellors

Position Summary

Reporting to the Manager of Wood’s Homes Schools, the School Support Counsellor I is responsible for front-line care of young people admitted to Wood’s Homes Educational and Day Treatment Services continuum of services. The School Support I is also responsible for working collaboratively with school personnel, providing in-school support needs and liaison with the young person, family, and community, and assists with administrative tasks as assigned by the Program Manager.

Duties and Responsibilities

In conjunction with Program Management implements activities, plans and routines that facilitate the maintenance of a milieu that fosters the development of appropriate levels of personal, social and educational functions for each student.

  1. In consultation with Program Management and leading personnel assures that an (IPP) individualized program plan includes both treatment and educational goals and that case management strategies are established for each student.
  2. Communicates with guardians, clinicians, social workers, school representatives and other individuals involved in case management. Communicates information concerning clients and families through monthly reports, clinical rounds, and team meetings.
  3. Shows developing skills in individual and group counselling, family liaison.
  4. Intervenes directly in crisis situations.
  5. Ensures the provision of necessary medical and clinical services (as prescribed by relevant professionals) for young people involved in a school program.
  6. Maintains up to date health and clinical records with the support of Program Management.
  7. Role models effective, appropriate and mature coping strategies for each student to utilize in his/her personal growth.
  8. Participates in case conferences, rounds, team meetings and supervision as established by Senior Management.
  9. Assists in feedback regarding practicum students.
  10. Attends designated conferences/seminars and professional training, and executes other responsibilities as assigned by Program Management.
  11. Works in accordance with the ethics and customs of the childcare profession and Wood’s Homes policies.
  12. Actively participates in supervision with principles, Educational and Day Treatment Services Program Management.
  13. Provides a liaison function to the young person, and their family, which assists them in developing and maintaining connections with their community.
  14. Executes other responsibilities as assigned.

Qualifications

Diploma/degree in Social Sciences, Child Care or related area and acceptable experience in working with adolescents and families. Must have own transportation. Personal suitability.
