Wood's Homes Programs: Home Connections
The Home Connections program is a family support and preservation program that provides in-home support services to children (ages 0-18) and families (guardians, caregivers, foster parents) in their homes and communities.
This program offers family driven initiatives to assist family members to remain together, improve family relationships, facilitate a child's return to home from alternate community placement, and strengthen family ties to the community.
The family, in-home support counsellor and referral source collaborate to identify goals that are concrete and a manageable. Goals are reviewed with the families on a regular basis to ensure goal attainment. Pre and post measures are utilized to assess family progress.
